Instructions to access your medical records online:

  1. Provide your email address to the Registration Desk either in the clinic or the hospital.

  2. You will receive an email invitation from Minneola Healthcare inviting you to join the HealthLife Patient Portal.  Accept the invitation, then answer the questions on the screen you will be shown.

  3. Verify your identity.  Enter information into the required fields and answer the security questions.  After reviewing everything, finish by clicking "Create Your Account".

  4. Follow the online prompts to finish creating your account.  Make sure you log in with a username and password that you will remember.

  5. Select a security question that can be used to verify your identity should you forget your login information.

  6. You're online!  You now have secure access to your medical information.  View your health information online at https://minneolahealth.iqhealth.com!
